Does Dr. Wages require a consultation? How much is it?

For all elective procedures Dr. Wages requires a consultation visit.  These appointments generally last 1-Hour and are $150.00. The $150 deposit is taken when you BOOK the appointment.  Should you decide to move forward with a procedure your $150 consultation fee will be applied toward your surgical deposit if you book within 30 days of your visit.

Do I need to bring my insurance cards to my appointment?


Why do I need to provide insurance information when I am here for an elective procedure?

We are required to have medical information for every patient on file since we are a medical office.  We also do this because you may come back one day for a non elective procedure, and if in the unlikely event you become ill or hurt at our office, it benefits everyone to know what insurance you have and who your primary care doctor is.

Why do I need to provide an ID?

Not only do we want to make sure we are treating the right Jane or John Smith, but it's nice to be able to put a face to the name.  Our system allows us to upload your picture so we know who you are before you get here.

How often do forms need to be updated?

Our patients medical history and demographic information need to be updated on a yearly basis.
